  INSTRUCTIONS


- Submission deadline is Friday, September 5, 2008, at 5 p.m.
- Nominations must be submitted using this form.
- Only information on the application will be judged. News articles, resumes, additional pages, or other supporting documents should not be submitted with this application.
- Complete the nomination form in as much detail as possible as judges will use only the information provided on this form as the criteria for judging.
- A copy of the nomination form will be given to the nominee in order to provide more details and to ensure accuracy.

CRITERIA


The ATHENA Award Program celebrates the potential of all women to serve as valued members and leaders of the community and recognizes those who support them. The ATHENA Award honors individuals who strive toward the highest levels of professional accomplishment - women and men who excel in their chosen field, have devoted time and energy to their community in a meaningful way, and who also open paths so that others may follow.

Recipients must meet each of the following criteria:
- Assist women in reaching their full leadership potential
- Demonstrate excellence, creativity, and initiative in their business or profession
- Provide valuable service by devoting time and energy to improve the quality of life for others in the community.

JUDGING


- A selection committee of community leaders and out-of-town judges will review all nomination forms and select the recipient.

20. BUSINESS ACCOMPLISHMENTS: List special recognition, projects, and achievements. Include business-related affiliations, directorships, trusteeships or other designations.
21. COMMUNITY SERVICE: Describe local, state and national level participation, length of service and offices held in civic and service organizations, charitable activities, political pursuits, religious groups, chambers of commerce, merchants' assoc., etc.
22. OTHER MEMBERSHIPS, ACHIEVEMENTS, AWARDS, PUBLICATIONS: List awards & honors received that support this nomination including other nominations, publications or articles published supporting their service to their profession, industry or community.
