Pearl of Promise Nomination Form

  INSTRUCTIONS


- Submission deadline is Friday, September 5, 2008, at 5 p.m.
- Nominations must be made by a teacher, guidance counselor or mentor.
- Nominations must be submitted using this form.
- Nominee must provide copy of transcripts.
- Nominee must write a 500- to 1000-word essay describing her career goal and how she is preparing herself to achieve it.
- Complete the nomination form in as much detail as possible. Judges will use only the information provided on this form and essay written by the nominee as the criteria for determining finalists. News articles, resumes, additional pages, or other supporting documents should not be submitted with this application.

CRITERIA


Recipients must meet each of the following criteria:
- Must be a senior planning to graduate from a Hamilton County public, private, or home-school in 2008.
- Must demonstrate a strong scholastic aptitude in math and science.
- Must excel in subjects or have interest in fields in which women have traditionally been underrepresented, such as one of the following:

1. Applied Mathematics - Concentrations: Actuarial Science, Corporate Science, Computer Science, Systems Science.
2. Architecture
3. Business Administration - Concentrations: Accounting, Entrepreneurship, Finance, General Management, Human Resources Management, Industrial Management, Marketing.
4. Biology - Concentrations: General Biology, Ecology, Molecular, Organismal, Preprofessional.
5. Chemistry
6. Computer Science - Concentrations: Computer Engineering, Information Systems, Scientific Applications.
7. Economics
8. Engineering - Concentrations: Chemical Engineering, Civil Engineering, Electrical Engineering, Engineering Management, Environmental Engineering, Industrial Engineering, Mechanical Engineering.
9. Industrial Technology Management
10. Environmental Science - Concentrations: Biology, Chemistry, Engineering Science, Geography, Geology, Mathematics, Physics, Sociology, Anthropology.
11. Geology: Concentrations: Geology, Environmental Geology.
12. Mathematics
13. Medical Technology
14. Physics
15. Political Science - Concentrations: American Studies, International and Comparative Studies, Legal Studies, Public Administration.
16. Secondary Mathematics
17. Secondary Natural Sciences - Concentrations: Biology, Chemistry, Earth and Space Science, Physics.

JUDGING


- A selection committee of community leaders and out-of-town judges will review all nomination forms and select finalists.
- Finalists will then be called to be interviewed by a panel of judges.

SCHOLARSHIP PROCESS


The recipient will receive a $2,500 scholarship to be used at any accredited college or university. Once enrolled in school, the recipient is obligated to contact the Chattanooga Area Chamber of Commerce so that we may pay the first installment of $1,250. The second installment of $1,250 will be paid once the student provides a copy of her transcripts to the Chamber showing that she has maintained a 3.0 GPA. If a 3.0 GPA is not maintained, funding will not be given the second semester.

21. LEADERSHIP SKILLS: Describe how the nominee has demonstrated leadership skills in the classroom, in extracurricular activities and in community and volunteer work.
22. ADDITIONAL INFORMATION: Describe in detail any additional qualities or activities you feel should be taken into account when considering this nominee.
