Stargazer Nomination Form
- Submission deadline is Friday, September 5, 2008, at 5 p.m.
- Nominees may be self-nominated or nominated by an outside source.
- Nominations must be submitted online or in an electronic format.
- Nominees must write a 500 to 1,000 word essay describing her career goals and how she is preparing to achieve them.
- Complete the nomination form in as much detail as possible. Judges will use only the information provided on this form and essay written by the nominee as the criteria for determining finalists. News articles, resumes, additional pages, or other supporting documents should not be submitted with this application.
Recipients must meet each of the following criteria:
- Must be a female, U.S. Citizen over the age of 25 by September 1, 2008.
- Must live or attend school in the Chattanooga Metropolitan Statistical Area. (The Chattanooga MSA includes Hamilton, Marion and Sequatchie counties in Tennessee and Catoosa, Dade and Walker counties in Georgia.)
- Initial application must be completed by nomination deadline, and may be completed by applicant or a third party. Essay must be submitted by applicant.
- Must not have been convicted of a felony in any U.S. jurisdiction within the three years prior to application.
- Must be officially accepted into an accredited degree or certificate program at an accredited U.S. college or university.
- Must provide copies of IRS Form 1040 from previous two years to document adjusted gross income.
- Must provide a copy of her most recent pay stub.
- Must be able to demonstrate financial need and not exceed total household income according to the chart below:

- A selection committee of community leaders and out-of-town judges will review all nomination forms and select finalists.
- Finalists will then be interviewed by a panel of judges.
- Chosen finalists will be required to undergo a background check.
- One recipient will be chosen to receive the $2,500 scholarship.
The recipient will receive a $2,500 scholarship to be used at any accredited college or university. Once enrolled in school, the recipient is obligated to contact the Chattanooga Area Chamber of Commerce to request payment of the first installment of $1,250 to the institution of choice.
The second installment of $1,250 will be paid to the institution once the student provides a copy of her grades to the Chamber showing that she has maintained a 3.0 GPA. If a 3.0 GPA is not maintained, funding will not be given the second semester.
Funds are not required to be used within the semester awarded; excess funds may be retained by the institution to be applied to future fees.
Forfeiture of Award
- Does not maintain a 3.0 GPA.
- Does not enroll at least part-time in the semester in which the award is to be in effect.
- Is placed on academic and/or disciplinary probation.
- Is convicted of a felony during the period of the award.
